HYDERABAD: It’s not rare that people who are diabetic experience symptoms like tremors, palpitations and anxiety- just in  the middle of an important presentation or while traveling or  may be at any other hectic day at work. Also, it is not rare  that they ignore it thinking it to be the usual day to day tiredness or, attribute it to the recent office/ family stress and walk away with self-treatment, little of rest and ignorance. These are classical symptoms of hypoglycemia. With repeated episodes of hypoglycemia, patient may not get any symptom and can go directly in coma!! This is called hypoglycemic unawareness. Often these people who are diabetic believe that ‘low blood sugar level’ is the last thing on the earth that can happen to them and that is the major reason that why these people on medication and  insulin for treatment do not find a reason to why should they be diagnosed for something like hypoglycemia. Not surprisingly, many of them have not even heard of anything like this.  CausesHypoglycemia can cause serious morbidity, provoking major vascular events such as stroke, myocardial infarction, acute cardiac failure, and ventricular arrhythmias if not treated on time and properly. Sometimes prolong neglected hypoglycemia can cause irreversible brain damage.  While mild hypoglycemia is usually defined by the ability to self-treat, episodes requiring external assistance are defined as severe hypoglycemia. The frequency of hypoglycemia has been examined most extensively in people with type 1 diabetes, in whom mild hypoglycemia occurs on average around twice weekly whereas severe hypoglycemia may lead to seizures and loss of consciousness, which require emergency treatment. Night hypoglycemia This very common in Type 1 diabetic patients and can manifest with bad dreams, inadequate sleep, restlessness and feeling of tiredness on waking up.Though the problem is big and scary, simple precautions to a great extent can help you identify and deal with this problem.  It is often advisable to the patients on diabetes medications or insulin that incase they are feeling nervous, anxious, shaky, tired, confused, unusually hungry, dizzy, or experiencing a  fast heart beat, it is time to visit the doctor and get a proper check-up.
